If you have been given a form to complete by a parent, please complete it and return it to the parent who gave it to you along with a copy of your registration or approval certification.

If you want to join the scheme and have not yet been approached by a parent, then all you need to do is complete this form and return it with a copy of your registration or approval certification to -
